Sadia.  Not clear about the hierarchical aspect. But, I'm not sure a mixed
logit
model is the right platform.  More like an ordered probit if severity is
discrete.
Regression model if continuous.  Also, it's not clear how a mixed logit
model
could be treated as a regression as appears to be suggested below.
