[Limdep Nlogit List] Generalized Random Thresholds Ordered Choice Model

Dear Limdep listers,

If it's duplicate, I'm sorry, but I checked the archive for 2010 and 
found nothing. I also checked the documentation (chapter E22 in volume 1 
of econometric modelling guide) and found nothing.

My intention is to estimated the model in Greene and Hensher (2010) 
Section 8.3 page 202. (This is the book: 
http://pages.stern.nyu.edu/~wgreene/DiscreteChoice/Readings/OrderedChoiceSurvey.pdf)

Or more specifically, I'm wondering what command produces output like 
what on page 206 of the above book, titled random thresholds ordered 
choice model.

Any help is greatly appreciated.
