[Limdep Nlogit List] ordered logit: one threshold missing?

Dear All,

I'm estimating the ordered logit model with a 4-level categorical  
variable as a dependent variable (coded 0,1,2,3). However, the output  
only reports two threshold values instead of three.

Has anyone had the same problem before, what could be the reason for  
this, and is there anything that can be done for this?

My command is:
ORDERED;Lhs=CERTAINT;Rhs=ONE,DIFF,TAX,TAXLOWER,ACCTAX,ACCSCE,TRUSTACT
     ,HIEDUC,GENDER,FREETIME;Logit$
